SportSlip provides physician-reviewed sports and camp physical clearances for kids ages 5–18 — entirely online, with no appointment or waiting room. Parents complete a structured screening built on the American Heart Association's 14-element cardiac history and the American Academy of Pediatrics' preparticipation evaluation (PPE-5), submit vitals and a short guided movement video, and upload the actual form their school, camp, or league requires. Every submission is personally reviewed and signed by Dr. Adam Z. Kawalek, MD, a board-certified internal medicine physician licensed in 29 U.S. states (NPI 1326223306) — not an algorithm, not a template. Most families receive their signed clearance within 24 hours. Clearances are valid for 12 months and cover every sport and season. If our screening finds something that needs in-person care, we say so — you get a referral letter and a refund, not a rubber stamp. SportSlip is HIPAA-compliant and a service of SickSlip Inc.

Had my daughter's sports physical done…

Had my daughter's sports physical done by them. Saved us a trip to a clinic. Process was thorough and the doctor reached out to us with follow up questions regarding my daughter's health. They also asked for a quick video exam and vaccination records. Felt reassuring. Easy and affordable.

Process was super easy and cheaper than…

Process was super easy and cheaper than getting my daughter into the dr because her sports physical was expiring prior to her next Dr. appt and they get really booked up in the summer for other kid's sports physicals.

I had a question and responded to very quickly. I would highly recommend using this service if you can't get into the Dr.

Health clearance

I was able to get a school (college) health clearance form filled out here on the same day I wrote them. They were so efficient and communicative. I was very impressed and am going to recommend them to my classmates/peers. Thank you!
