Providing older people with a helping hand when they need it most since 1966. Reducing loneliness amongst isolated and lonely older people by providing befriending volunteers and FREE services that offer emotional and practical support to vulnerable people. Working closely with health professionals across Sheffield and Doncaster to help older people live independently and improve their well-being and quality of life. We offer a number of services including our Befriending service the Good Neighbour Scheme where volunteers visit isolated older people in their homes for a cuppa and a chat. We also offer telephone support for those who do not want face-to-face visits and a pen pal scheme for those living with hearing impairments. Our Hospital-to-Home service assists vulnerable older people whilst in hospital, upon discharge from hospital and in their own homes, helping with the delivery of mobility aids, emergency food shopping, and transportation home or to hospital appointments. Our Doncaster-based Home from Hospital Scheme and Sheffield-based Back Home Schemes are intensive four-week personalised services where patients are given support and guidance, helping to increase their confidence and reduce readmission to hospital. Placement Support enables older people who can no longer live safely at home to reassess their housing options. Our friendly staff will work with them to find a solution and work through the process. I work with a team of community nurses, we use SCCCC's almost daily. If it wasn't for your service I'm not sure what we would do, everyone is always so friendly and helpful. I really do think you help prevent hospital admissions and keep our lovely patients safe in their own homes.

Love this company and the service they provide. Always go above and beyond for the older people of Sheffield. We always have a little groan when demesne lives out of the city as things are so much easier with the help of SCCCC ! OTs who work elsewhere are very jealous of the fact we have these marvellous people around

Brilliant service has helped a lot with delivering equipment and supporting patients with shopping I have used S4Cs many times for supporting discharge and for patients in the community. The staff are friendly and will direct you to the appropriate member of staff when called.

I am a therapist and a manager within the NHS and have been working with S4C's for almost a decade now. I cannot recommend the good work they do enough. I have never seen a group of people work so hard and go above and beyond to try and create amazing outcomes for people.

I have been a volunteer with this charity for a couple of years now,they have paired me up with a lady who has become a true friend and my experience with S4C guidance and support has been incredibly kind,compassionate and thoughtful but they not only support me as a volunteer,they are also there for my friend and be they have helped her with transport and also the real kindness of sending a card every now and then so she knows they are there and she isn’t alone now that she’s more or less housebound.I would recommend this charity to anyone looking for a volunteering job or if a loved one needs some support.
