I’m so glad that my company uses Form. They have been instrumental during my weight loss journey. I highly recommend them to anyone as a resource to guide you along the way. They get a 10 out of 10!

Unfortunately, I am unable to provide a positive review so far. As a new subscriber, I have been trying to connect with the Form Health team for over a month leaving 4 messages within the last week and a half with no return call. I am beyond frustrated but am stuck using this service for now.

I have been working with Form Health for 2+ years and would absolutely recommend them to anyone who desires to lose weight — and keep it off — in a healthy manner. This program helped me lose 30+ lbs and I am grateful for the accountability and cheerleading via convenient and regular virtual check-ins with my dietitian, PA and MD. They are superstars who have equipped me with the tools I needed to get to a healthy weight and improve my fitness level.

The program has been awesome!! The Form Health team are all awesome! They are very helpful. The app is awesome and love that the appointments are virtual and the ease of the app is a Plus!!! I would recommend this program to everyone due the ease of the app and the professionalism the Form Health team!!

There’s never a moment I felt I was on my own for care. Immediate action is taken any time I ask a question. Both my physician and dietitian are amazing!

The Form Health team has been an amazing. It is a great resource to have these individuals all working together for the healthiest outcome. This has been a top tier experience and it is certainly one that I am thankful for.

The app doesn’t actually help you do anything. You have to talk to the support team to do everything. Most of the doctors are rude and dismissive.

All of the help and guidance I have received has been very helpful.

The doctor and the dietician are knowledgeable, helpful, and compassionate.

I appreciate the help I get from dietician and the dr but it would be so much better if I were with my family doctor, as I did prior.

My nurse practitioner is so warm and kind, she is extremely helpful and encouraging. I appreciate that she gave me a coupon to make my treatment free. I am someone who hates anything to do with medical visits but I look forward to having my meetings and going over my progress.
